Founded in 1972, DSC provides educational, professional, residential and behavioral support to people with intellectual and developmental disabilities in Champaign and Ford counties. RESPONSIBILITIES:

  • As a Health Advocate for DSC’s Community Living program, you will be responsible for coordinating and providing appropriate and timely planning for individuals’ routine medical, dental and psychiatric appointments for people participating in the Community Living program.  You will also provide transportation to appointments and other diagnostic services as needed, including support during times of emergent illness/injury. Additionally, the Health Advocate provides documentation of clinical appointments and communicates pertinent information to team members. Each day, you will play an integral role in the success of the company and our mission to support people to live a rich and meaningful life.
REQUIREMENTS:
  • High School diploma or equivalent.
  • Sufficient physical stamina and mobility for lifting, carrying, pushing and pulling.
  • Experience supporting people with intellectual and/or developmental disabilities preferred.
  • Ability to complete agency required training.
  • Valid driver’s license and personal vehicle to provide daily transport for people to appointments.
  • Ability to drive an agency van.
HOURS:
  • 40 hours a week, Monday – Friday, typically 8:00 AM – 5:00 PM
