DSC provides professional, residential, person-centered support to people with intellectual and developmental disabilities in Champaign and Ford counties. If you are seeking a fulfilling job, we are currently hiring for the position of Medical Coordinator

Overview:
The Medical Coordinator is responsible for coordinating medical supports for people in the CILA program under the direction of the RN Coordinator. This position embraces DSC’s Mission and incorporates these values into the everyday activities, culture, and opportunities offered to people supported. The Medical Coordinator will ensure individuals experience their Best Possible Health. The Medical Coordinator is required to work collaboratively with the RN-T and possess the ability to adapt and quickly change priorities to meet the needs of individuals. This position will share responsibility within the Residential Team to ensure compliance with DHS Rules 115 and 116.

Responsibilities:
• Serve as a resource to ensure all nursing needs are met as required by Rule 115 and Rule 116.
• Works with the RN-T Coordinator, for the completion and oversight of annual nursing assessments, TD screenings, medication/medical records, Medication QA, and other required documentation.    
• Serves as a liaison to individuals/guardians and physicians to streamline timely communication.
• Attend team meetings as needed for all medical concerns. This may be person specific or site specific. 
• In the absence of the RN Coordinator, provides consultation and access to consulting RN as needed regarding the health and medical issues relevant to care and treatment of individuals served in the CILA program.
• Works with medical providers to ensure orders are received for all medications.
• Maintains communication with the team on assuring all follow-up is completed for ED visits, hospitalizations, urgent care visits, etc. 

Requirements:
• Work experience in a medical or human services setting.
• Knowledge and/or experience working with individuals with IDD.  Ability to work cooperatively with staff, agency, and community personnel.  Ability to independently structure schedules, both personal and program, according to priority needs. Ability to guide professional and para-professional staff to support individual and agency outcomes consistent with DSC’s Mission.  Demonstrate good interpersonal and organizational skills.
• Must successfully complete all required training within 120 days from date of hire.
• Proficiency in Microsoft office.
• Physical Requirements – Sufficient physical stamina and mobility for lifting, carrying, pushing, pulling. Ability to lift 50 pounds. Ability to stand/walk for up to 2 hours at a time. Ability to perform CPR.
• Transportation Requirements - Valid driver’s license, proof of insurance, and access to reliable transportation.

Additional Desirable Qualifications:
• QIDP-certified or QIDP-eligible.

Hours:
• Typically 8:00 AM - 5:00 PM, 40 hours per week with some availability necessary after hours for emergencies.

• Grant-funded retention bonuses up to 2 payments for a maximum of $1,200 per fiscal year! (depending on when in the grant year you were hired)
• Hourly range between $21.64 -$ 24.04
