DSC provides professional, residential, person-centered support to people with intellectual and developmental disabilities in Champaign and Ford counties. If you are seeking a fulfilling job, we are currently hiring for the position of Medical Coordinator.

Overview:

  • The Medical Coordinator is responsible for overseeing all medical supports for people in the Community Integrated Living Arrangement (CILA) program. This position embraces DSC’s Mission and incorporates these values into the everyday activities, culture, and opportunities offered to people supported. The Coordinator will assure individuals experience their Best Possible Health. The Medical Coordinator is required to work independently and possess the ability to adapt and quickly change as advances in the field and agency occur.

RESPONSIBILITIES:

  • Serves as main contact with nursing consultant to assure all nursing needs are met as required by Rule 115 and Rule 116.  This includes medication change forms as well as training needs for DSPs.
  • Works with consultant, for the completion and oversight of annual nursing assessments, TD screenings, medication/medical records, Medication QA, and other required documentation. 
  • Responsible for completing the required Health Risk Screening Tool (HRST) annually and as needed.
  • Coordinates clinics and hospital support/services such as routine visits, emergency access, weekly lab draws, and other specialists as needed for people to receive the medical attention they need.
  • Attends critical medical appointments to assist team in accurately communicating concerns for people supported.
  • Serves as a liaison to individuals/guardians and physicians to streamline timely communication.
  • Attends team meetings as needed for all medical concerns. This may be person specific or site specific.
  • Coordinates the purchase, ordering, receiving, and storage of medications and medical supplies.
  • Coordinates and serves as a backup for administering medications to individuals as prescribed in CILA setting and/or midday medication passes for individuals in Community Day Services. This includes reviewing physician’s orders, medication sheets and medications.
  • Responds and facilitates support for emergency situations.
  • Provides consultation and access to consulting RN as needed regarding the health and medical issues relevant to care and treatment of individuals served by DSC.
  • Maintains ongoing communication with Pharmacy to assure all medication orders occur, and there are no gaps in delivery of medications to people supported.
  • Monitors tracking system for all individual specific medical needs, and assures departmental compliance, policies, and procedures per Rule 115 and Rule 116.
  • Conducts periodic in home medication audits to assure medication systems are in place.
  • Works with medical providers to assure orders are received for all routine medications and prn medications.
  • Supervises, supports, and trains Health Advocates.
  • Mentors the team on assuring all follow up is completed for ER visits, hospitalizations, urgent care visits, etc.
  • Works with the team to assure compliance with CMS standards as well as BALC, BQM, OIG, DCFS, CQL etc. are consistently met.
  • Provides representation on agency committees as needed.

REQUIREMENTS:

  • Bachelor’s degree in human services field (Sociology, Special Education, Rehabilitation Counseling, Psychology, etc.)
  • Work experience in a medical or human services setting .
  • Must successfully complete all required training within 120 days from date of hire.
  • Sufficient physical stamina and mobility for lifting, carrying, pushing, pulling. Ability to lift 50 pounds. Ability to stand/walk for up to 2 hours at a time. Ability to perform CPR. 
  • Valid driver’s license, proof of insurance, and access to reliable transportation.           
  • QIDP-certified or QIDP-eligible
  • Additional desirable qualifications include experience working with people with intellectual and developmental disabilities, supervisory experience and experience using Microsoft Word, Outlook and Excel.
  • Full COVID-19 vaccination required.

HOURS:

  • Typically 8:00 AM - 5:00 PM, 40 hours per week with some availability necessary after hours for emergencies.
