DSC provides educational, professional, residential, employment and behavioral support to people with intellectual and developmental disabilities in Champaign and Ford counties. If you are seeking a rewarding and fulfilling job, we are currently hiring for the position of Community First Coordinator.

 

RESPONSIBILITIES:

As a Community First (CF) Coordinator at DSC, you will assist the Manager of CF in coordinating tasks for the CF program. In this role, you will support the daily operation and development of the program as well as assist in training of CF staff. You will manage a caseload, which includes the development, implementation, monitoring, and modification of individual outcomes consistent with the Individual Personal Plan. Additionally, you will assist in the development/scheduling of community groups and provide on-going monitoring of the activities to include individual documentation and outcomes data.

 

REQUIREMENTS:

· High school diploma or equivalent

· Excellent communication and organizational skills

· Fundamental computer skills, including general web browsing and Microsoft Office Suite (Outlook, Word, Excel)

· One or more years of experience working with individuals with developmental disabilities desirable

· Sufficient physical stamina and mobility for lifting, carrying, bending, pushing and pulling. This may apply to the support of individuals and/or the products/materials related to the support of individuals

· Valid driver’s license and the ability to drive an agency van.

 

HOURS:

· 40 hours a week, Monday – Friday
